Pain and suffering is a physical mob located in at least 1 zone that I've found (Lavastorm Mountains). You can F8 around and you'll eventually see the health indicator on your Target bar fill up, but no name will be present [Somewhere in the NE third of the zone, I think at the bottom of a pool of lava -- this was in classic, decades ago - I haven't verified this on p99]. I fought (and lost) to him in Lavastorm, as he was around level 50 if memory serves.

I'm not sure how many HP in the negative you need to be to physically die, but I think anywhere from 0 HP to [Loading, please wait], is a time where Pain and Suffering can actually kill you.
