In a tiny room slightly bigger than a cupboard, on its own (although a roaming skeleton or ghoul may drop by). Loc is pos 496, pos 46, pos 19.
Top of the Southeast tower (the stairs are opposite the tiny room)

Confirmed 4/29/22 that all three spawn points are on a 12 min spawn, unlike the rest of the dungeon.
