A placeholder is a mob that has spawned in the place of another named mob. For example a frenzied ghoul can be placeheld by a bloodthirsty ghoul.
Every time the mob spawns, it will randomly spawn as either the placeholder or as the named mob. The chances for the named mob to spawn are often much lower than for the placeholder.
In most cases when camping a named mob, it is important to kill the placeholder as soon as it spawns. This allows it to respawn as quickly as possible, giving it a chance of coming back as the named mob.
