- Upper Portion: Found via the Niblek's Gems quest in Chardok. (bring Niblek a Black Sapphire and a Ruby)
- Middle Portion: Dropped by Verix Kyloxs Remains, found in the sewers of Karnor's Castle.
- Bottom Portion: Found sitting on the floor in the library in Kaesora.

Remember that there are three separate medallions in the game, each broken into three pieces that just say a piece of broken medallion when you find it. You need to identify the piece of medallion you find to determine which one it is.
